Inclusion and Exclusion Criteria

- Exclusion Criteria:
- Severe forms of extrapulmonary TB, specifically TB meningitis, osteoarticular TB, or disseminated/miliary TB with multiorgan involvement.
- Pregnant or breastfeeding adolescents.
- Patients with prior exposure exceeding one month to Bedaquiline, Linezolid, or Pretomanid/Delamanid without explicit drug susceptibility testing confirming ongoing sensitivity.
Dosing Schedule (Adolescents $\ge$ 14 Years)
- All medications are administered orally under Directly Observed Treatment (DOT) for exactly 26 weeks (6 months).
- Bedaquiline: Loading dose of 400 mg once daily for the first 14 days, followed by a maintenance dose of 200 mg three times per week.
- Pretomanid: 200 mg once daily.
- Linezolid: Initiated at 600 mg once daily. Dose is reduced to 300 mg daily if severe myelosuppression or neuropathy develops.
- Moxifloxacin: 400 mg once daily.
- Adjunctive Therapy: Mandatory co-administration of Pyridoxine (Vitamin B6) at 50-100 mg daily for the entire duration to proactively mitigate Linezolid-induced peripheral neuropathy.
