Introduction And Epidemiology

- While classically a benign childhood disease, it presents with significantly higher morbidity, mortality, and complication rates when it occurs in adolescents, defined roughly as 10 to 19 years of age.
- In tropical and subtropical regions, the peak age of infection is frequently delayed, resulting in a higher proportion of susceptible adolescents who are at greater risk for severe disease.
Pathophysiology And Transmission
- VZV is highly contagious and spreads primarily via aerosolized respiratory droplets or through direct contact with vesicular fluid.
- Patients remain infectious from 24 to 48 hours prior to rash onset until all skin lesions have completely crusted over, typically 4 to 7 days after the rash appears.
- Following inhalation, the virus replicates in the upper respiratory tract and regional lymph nodes, constituting the primary viremia.
- Subsequent secondary viremia disseminates the virus to the reticuloendothelial system, skin, and viscera.

Clinical Manifestations
Prodromal Phase

Exanthematous Phase
- The rash begins as intensely pruritic erythematous macules that rapidly evolve into papules, followed by clear, fluid-filled vesicles classically described as a dewdrop on a rose petal.
- Vesicles become cloudy and pustular within 24 to 48 hours, eventually umbilicating and crusting.
- The distribution is characteristically centripetal, originating on the scalp, face, and trunk before spreading to the extremities.
- A hallmark feature is cropping, defined as the simultaneous presence of lesions in all stages of evolution within the same anatomical area.
- Adolescents consistently develop a higher burden of lesions, frequently exceeding 500, with new crops continuing to appear for more than 7 days.
Complications

Management Protocol
Pharmacological Therapy
- Antiviral therapy is universally recommended for all unvaccinated adolescents over 12 years of age due to the elevated risk of severe disease.
- Therapy must be initiated within 24 hours of exanthem onset to ensure clinical efficacy.
